                                             (Community Alternative Program)

 

1. Day Supports

Assistance with acquisition, retention, or improvement in: Self help - Socialization - Adaptive skills take place in non-residential settings, separate from the home or facility in which the individual resides.

2. Home and Community Supports

Intended to meet the habilitation and support needs in an individuals own home or in the community which are described in the 'Plan of Care' for the consumer.

Provides instruction and assistance with:

-Acquiring skills - Maintaining skills - Functioning with greater independence in the home & community - Supervision & Monitoring

-Development of personal relationships & interpersonal skills

3. Personal Care

Includes support, supervision, and engaging participation with:

-Eating/preparation of meals - Bathing - Personal hygiene - Dressing - Bed making

Monitoring:

-Health status - Physical condition - Use of special mobility devices

(Does not include medical transportation)

4. Residential Supports

Provides the habilitation and personal care needs of individuals living in:

-Licensed residential settings - Unlicensed alternative family living homes (AFL Providers)

Assistance with:

-Acquisition - Retention - Improvement

Expectation of services include:

-Support - Supervision - Engaging participation

5. Respite

Provides periodic relief for the family or primary caregiver.

Provided in the individuals home or in an out-of-home setting/foster home.
