Le'Chris Services - Community Support Services (CS)

Community Supports started on March 20, 2006. This new enhanced service replaced Case management and CBS (Community Based Services) mentoring. Community Support can be provided in the home, community, or school.

Community Support offers special assistance in the following ways:

-Individualized Mentoring - Management Programs - Skill Development Programs - Activity Centers - Summer Challenge for young people and their mentors - Family Enrichment Program

-Communication Skills - Coping Skills - Self-Care Skills - ob Skills - Self-esteem

SPECIAL NOTES ABOUT COMMUNITY SUPPORT SERVICES

Community Support must be found medically necessary by a Doctor or Nurse Practitioner.

Community Support is primarily funded through Medicaid but not in every area. If no Medicaid is available then the consumer would go through the local LME to seek Community Support - Please ask at your local Le'Chris office about this detail.
