Case Management has recently been re-added to the service array available to clients within the mental health system and replaces some of the services formerly provided by Community Support.

Case Management is an activity that assists clients with gaining access to necessary care appropriate to their needs such as medical, behavioral, social and other services appropriate to their needs.  Case Management is individualized and person-centered and empowers the client to regain control of their life. 

The Case Manager is the primary point of contact in coordinating the multiple services that a client might need to be successful.

The functions of Case Management include:

  • Conducting a comprehensive assessment of the needs of the individual - those things that prevent the client from regaining or maintaining their mental health or substance abuse goals.  The assessment also identifies the strengths that the client possesses and his preferences for his treatment.

  • Person-Centered Planning - Developing with the client and their natural and/or paid supports a plan defining the client's goals and how all supports an assist the client in reaching these goals.  The plan provides self-direction to the client as he has participated in its development.

  • Referral/Linkage - Referring the client to available supports and/or services in the community.  These might include:

    • Referrals for medical or psychiatric care

    • Referrals to mental health therapists and/or substance abuse specialists

    • Referrals to other mental health services such as Psychosocial Rehabilitation, Community Support Team, ACTT, Day Treatment for Youth, etc.

    • Referrals to peer support programs

    • Identification and assistance in helping the client build his own natural support system

    • Referrals to Social Security, DSS, and other financial entitlement programs

    • Referrals to housing agencies, HUD, homeless shelters, available low-cost housing, etc.

    • Referrals to domestic violence agencies

    • Referrals to food banks, soup kitchens or other assistance agencies

    • Referrals to child services such as Boys and Girls Clubs, Scouting

    • Linking to faith-based groups and/or churches

    • Linking to Vocational Rehabilitation or other supportive employment programs

    • Linking to educational programs such as GED or Adult Basic Education classes for adults and to in-school educational opportunities for children.

    • Referrals to leisure/social opportunities such as recreation centers or special interest activity groups.

  • Monitoring and Follow-Up - After linking a client to one of the above or any other service, the case manager will follow up to see if the service is being delivered as needed and monitor if the service is assisting the client towards his goals.
